How to request legal records
Do you have a subpoena?
If you are an attorney requesting copies of our records where we are not a party to the lawsuit, a valid subpoena is required.
Subpoenas issued by a Michigan court must include:
- Member, health care provider name, employee or other entity name
- Contract or enrollee ID number
- Time frame
- Case name and number
- Appropriate signatures
Subpoenas for member records must be accompanied by these completed forms:
- Authorization for Use and Disclosure of Protected Health Information (PDF)
- And, if applicable — Authorization for Use and Disclosure of Psychotherapy Notes (PDF)

Pricing and response time
For each records request, we charge a minimum of $34 ($8.50 per hour, minimum of four hours) plus 25 cents per page. We will respond to complete and valid subpoena requests within four to six weeks of receipt.
Out-of-state subpoenas
