Care management programs

Our Care Transitions team reaches out to lead discharge planning for members with significant health issues. Members may be placed into a treatment plan such as a home-visiting provider program, high risk condition management, or advanced intensity case management. BCBSM may contact you to help coordinate care for these members.

Health and wellness
Our health care management program, BlueHealthConnection^® gives members health information to help them understand their health care issues, address their concerns, and work more closely with their providers.

• Members can view online articles, tools and quizzes that provide information on thousands of topics.
• Any time, day or night, members can speak with health coaches who may provide health information and answer questions.

Refer members to these programs by having them visit the Health assessment & wellness screen.

We encourage health care providers to refer members to these programs when appropriate.

Polypharmacy Part D program

Our Polypharmacy Part D program helps prescribing physicians identify opportunities to improve safety, decrease drug costs, and maximize patient quality of life for members who suffer from multiple chronic diseases and receive multiple drug treatments.
