Master Medical Claims
Master Medical coverage is available to all members with Blue Traditional coverage. Master Medical helps cover a portion of the out-of-pocket costs you would incur for services not covered by your Basic coverage.Because Master Medical payments are sent directly to you or the primary cardholder, your doctor may ask you to pay for services as they are received. You will receive payment once we process your claim and itemized receipts.
Submit the Master Medical Claim Form (28K PDF), with itemized receipts, to request reimbursement for paid services.
