AIM Toolkit for Providers

The 2007 Providers Immunization Toolkit for Children & Adults is a resource guide containing the most current standards of practice, forms, & vaccination methodologies for immunization in Michigan. Blue Care Network is a sponsor.


Alcoholism support

"Helping Patients Who Drink Too Much — A Clinician's Guide" distributed by the U.S. Department of Health and Human Services — National Institute on Alcohol Abuse and Alcoholism (NIAAA). Includes support documents to help physicians with alcohol screening, brief interventions and follow-up patient care.


Domestic Violence Toolkit

A 1999 JAMA study found that 90 percent of primary care physicians fail to screen their patients routinely for partner abuse during regular office visits. As providers, you play a crucial role in domestic violence intervention.


This nationally recognized toolkit offers practical tools to help screen, identify, refer and document confirmed or suspected cases of domestic violence.



Fighting childhood obesity

The Blues have developed the Good Health Club Physician Toolkit to support you in preventing obesity in your pediatric patients. This toolkit provides materials that encourage children and young adults to develop healthy lifestyles.


The toolkit is based on materials created here in Michigan and is now being used by Blue Cross Blue Shield providers across the nation. It was developed in consultation with the American Diabetes Association and the American Academy of Pediatrics.


The following items were created for your reference and to share with your patients and their parents. Feel free to download and print them.

Pediatric Healthy Weight Toolkit

Some projections hold that today's children may be the first generation in history to have shorter lives than their parents. The Blues have developed this toolkit in light of the epidemic of childhood obesity, and as part of our unique mission to improve the health status of Michigan residents. We hope it will help you take a more active role in assisting pediatric patients to achieve a healthy weight, with the help of their parents.


The materials in the toolkit will help you:


  • Identify overweight or obese patients and recommend treatment interventions
  • Track patient progress over time using age-appropriate BMI charts
  • Diagnose and treat patients with comorbid conditions
  • Use sensitivity and empathy in communicating with patients and their families, to inspire positive behavior change
