Organized Systems of Care
Blue Cross Blue Shield of Michigan is working collaboratively with physician organizations across the state to develop an Organized Systems of Care Program. This program takes BCBSM's Patient Centered Medical Home Program to the next level, expanding it to include hospitals and specialists. Together, primary care physicians, specialists and hospitals will work to coordinate services across the health care continuum for a defined patient population.
Both the OSC and PCMH programs are an integral part of BCBSM's Physician Group Incentive Program. Since its inception in 2004, PGIP has facilitated practice transformation through a wide variety of initiatives. Physician organizations that participate in PGIP are rewarded for improved performance in health care delivery.
